Lower Eyelid Surgery Before and After

Lower eyelid surgery can reduce under-eye bags and help the eyes look more awake while still keeping a natural look. In this case, a man in his mid 60s is shown six months after lower blepharoplasty with lower eyelid CO2 laser treatment. No fillers were used. His goal was simple: reduce the bags under his eyes without making his face look changed or overdone.

Lower Eyelid Surgery Case: What We Treated

Before surgery, this patient had large bags under both eyes. These bags can make a person look tired even when they feel rested. For this case, we treated the lower eyelids with bilateral lower blepharoplasty and added CO2 laser treatment to the lower lid skin. The video shows the change six months after surgery.

The goal was not to change the patient’s face. It was to soften the heavy under-eye area and help him look more rested. At Boston Eyelid Surgery, we plan lower eyelid surgery around each person’s eye shape, skin, and goals. A good result should still look like the same person.

What Does Lower Blepharoplasty Do?

Lower blepharoplasty is surgery for bags and puffiness under the eyes. Depending on the patient, the surgeon may remove or adjust fat that is pushing forward in the lower eyelid area. The plan may also include steps to support the eyelid or treat extra skin when needed. The exact approach is chosen during a personal exam.

In this patient, the biggest concern was the strong under-eye bags. After surgery, the lower eyelid area looks flatter and less heavy. This helps the eyes look more open and awake. The change is clear, but the patient still looks like himself.

Why Add Lower Eyelid CO2 Laser?

Lower blepharoplasty treats the deeper bag under the eye, but it does not fix every skin concern on the surface. CO2 laser treatment can be used to improve fine lines, texture, and sun damage in the skin around the lower lids. Our laser treatment page explains the skin resurfacing options we use around the eyes. In the right patient, surgery and laser treatment can work together to create a smoother result.

Six Months After Lower Eyelid Surgery

At six months, this patient has a much smoother under-eye area. The large bags seen before surgery are much less visible. His eyes look more awake and the lower lids blend more evenly into the upper cheek. Most important, the result does not look forced or unnatural.

Six months is also a useful time to look at a surgical result because swelling has had time to settle. Healing is different for every person, so one patient’s result does not predict another patient’s result. Our before-and-after photo gallery shows many lower blepharoplasty results across different ages and treatment plans. Looking at many cases can help patients understand the range of possible outcomes.

Was Filler Needed in This Case?

No filler was used for this patient. His six-month result came from lower eyelid surgery and lower lid CO2 laser treatment. Some patients may need filler to soften a hollow area or blend the lower eyelid into the cheek, but it is not needed in every case. The right plan depends on the shape of the eyelid, cheek, and surrounding tissue.

This is why we do not use the same plan for every person. One patient may need surgery only, while another may benefit from surgery plus laser, filler, or another step. A careful exam helps us decide what is needed and what is not. The goal is to use the right amount of treatment for a natural result.

Recovery, Anesthesia, and Cost for This Patient

For this case, the expected downtime was about two weeks. The procedure could be done with general anesthesia or IV sedation, based on the surgical plan. Patients may have swelling and bruising during the early part of healing. Recovery can vary, so each patient receives instructions based on the exact procedure performed.

The cost for this patient was $10,500, including anesthesia, at the time of service. Pricing can change over time and may be different if other steps are added. Is Lower Eyelid Surgery Right for You?

Lower eyelid surgery may be an option if under-eye bags make you look tired or older than you feel. It may also be helpful when the bags stay present even after good sleep and healthy habits. Some people are better served by a non-surgical option, while others may benefit more from surgery. The best choice depends on your anatomy, skin, health, and goals.

At Boston Eyelid Surgery, we focus on results that look refreshed, balanced, and natural. Patients can meet with us at our Wellesley office to review their concerns and learn which options may fit. We will also explain recovery, anesthesia, cost, and the limits of treatment before you decide. There is no one-size-fits-all eyelid plan.

A Natural 6-Month Result

This patient’s case shows how lower eyelid surgery can reduce strong under-eye bags while keeping the face natural. Six months after lower blepharoplasty with lower lid CO2 laser, his lower eyelids look smoother and his eyes look more awake. No fillers were used in this case. The result is a good example of a noticeable change that still looks like the same person.

If you are bothered by under-eye bags, we can help you learn which treatment options may make sense for you.
